Summary
The job of the Lead Personal Agent is to review, edit, and approve Personal Agent (PA) work, which includes customer goal surveys, progress notes, customer budget/provider tracking, and individual service plans, for compliance with state, Medicaid and MOMVB requirements. The Lead Personal Agent also manage a caseloads of customers; typically 15. Interview customers and their families to determine customer needs, develop and monitor plan of service, make appropriate referrals, provide fiscal and contract writing support, maintain caseload records of evaluation, service provisions and other related information and documents.
Responsibilities
* Supervise Personal Agents * Analyze data, prepare reports and provide statistical information as needed * Work with QA Analyst to develop and monitor QA plan for MOMVB, and to implement processes/procedures * Participate in CDDP/Brokerage meetings, proactively, professionally representing MOMVB * Provide leadership by working effectively and professionally with county liaisons/SOC's to identify resources and opportunities, and problem solve difficult cases * Work with Personal Agents (PA's), Director, and Business/Resource Manager to develop and sustain a sufficient and diverse provider pool * Facilitate the development and utilization of community resources through education, training and networking * Act as Authorized Designee for Criminal History Check process, to facilitate efficient processing of provider Criminal History Checks * Other duties as assigned
Ideal Candidate
**Minimum Requirements:** * An undergraduate degree in a human services field and at least 3 years of case management experience. * Brokerage knowledge and knowledge of the public service system for developmental disability services in Oregon required **Preferred Characteristics:** * Supervisory experience is highly preferred * Excellent written and oral communication skills * Strong interpersonal skills and experience in mediation or conflict resolution * Proficiency with Microsoft Office computer programs * Proficiency in basic math skills, and the ability to apply them to budgetary and other planning functions
