Summary
Performs direct patient care activities for all age groups to include assessment, planning, implementation, and evaluation. Monitors and coordinates patient care activities with ancillary departments to optimize patient care and comfort. Functions within the scope of nursing preparation and consistent with the established guidelines of nursing practice, the goals of Mercy Ada, the Philosophy of the Sisters of Mercy, the Oklahoma Nurse Practice Act, the Code of Professional and Business Ethics and the Co-Worker Code of Conduct. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Responsibilities
**Assessment of the Patient:** 1.1 Recognizes a life threatening/emergency situation and responds quickly. 1.2 Assesses patients ability to understand 1.3 Uses physical assessment skills 1.4 Interprets and utilizes diagnostic and laboratory data 1.5 Identifies psychosocial and spiritual needs 1.6 Interprets information from monitoring technology correctly 1.7 Determines ability to perform activities of daily living **Planning of Patient Care:** 2.1 Selects appropriate Clinical Pathway 2.2 Develops and documents an individualized plan of nursing care from each patient assessment which addresses all current health problems requiring interventions during the current admission. 2.3 Reviews the plan of nursing care/clinical pathway at least every shift and documents additional problems/variances as identified. 2.4 Begins discharge planning on admission **Implementation of Patient Care:** 3.1 Provides patient care according to written plan of care/clinical pathway. 3.2 Provides patient care according to established policies and procedures. 3.3 Utilizes critical thinking skills to meet the patients physical needs, as possible. 3.4 Utilizes critical thinking skills to meet the patients spiritual and psychological needs, as possible. 3.5 Demonstrates technical skills necessary to function in nursing practice. 3.6 Uses organizational skills and sets priorities in providing care. 3.7 Integrates teaching in all aspects of care. 3.8 Addresses call lights and alarms in a timely manner. **Evaluation of Patient Care:** 4.1 Evaluates patients response to care and progress toward goals. **Communication First Touch:** 5.1 Utilizes First Touch communication techniques in all interactions. 5.2 Keeps patient and authorized individuals informed on aspects of care throughout hospitalization. 5.3 Gives report in concise and pertinent manner using SBAR format. 5.4 Communicates necessary information to physician utilizing read-back when appropriate and follows-up as needed. 5.5 Keeps shift coordinator informed of changes in patients condition. **Leadership Skills: Demonstrates skills as a team leader** 6.1 Delegates tasks appropriately and effectively. 6.2 Follows up to see that delegated task are carried out as assigned. 6.3 Demonstrates patient advocacy skills. **Leadership Skills: Demonstrates skills as a Preceptor/Resource Person** 7.1 Is a good clinical role model for new nurses. 7.2 Seeks out learning experiences for orientees. 7.3 Provides an environment that is conducive to learning. 7.4 Assists the orientee to perform new procedures and makes appropriate clinical judgments. 7.5 Is available to answer questions. 7.6 Makes the orientee feel part of the team. 7.7 Handles new co-worker problems appropriately and effectively. 7.8 Evaluates the progress of the new orientee. 7.9 Completes necessary documentation for the orientation process. **Leadership Skills: Demonstrates skills as a Shift Coordinator** 8.1 Makes assignments according to patient needs, staff strengths, and workload distribution. 8.2 Coordinates staffing for oncoming shift. 8.3 Oversees the orientation process for new co-workers (s) on shift. 8.4 Responds and provides leadership in emergency situations. 8.5 Monitors staff compliance with regulatory standards, nursing policies, Health System policies, and departmental policies. 8.6 Keeps Nursing Director and/or Nursing Supervisor informed of continued problems or significant issues. **Patient Safety:** 9.1 Recognizes patient safety needs. 9.2 Provides safe environment for patient. 9.3 Provides patient care according to established National Patient Safety Goals. 9.4 Implements and documents fall precautions according to Fall Risk Assessment/Fall Prevention Policy 9.5 Utilizes patient lift/transfer equipment appropriately and safely.
Ideal Candidate
*Education:* Graduate of a state approved professional nurse program. *Licensure:* Current RN licensure with the Oklahoma State Board of Nursing or current RN Oklahoma temporary work permit. *Experience:* One year of clinical nursing experience. *Certifications:* Must have a current American Heart Association Healthcare Provider card or complete a course within their probation period. *Other:* Working Conditions, Mental and Physical Requirements: Stand and walk most of the time on duty. Meet unit specific physical demands. Significant amount of bending and stooping, as well as lifting and turning patients with assistance. May be required to work differing schedules, shifts, days of the week, or work frequency according to patient care requirements. High risk exposure to blood and body fluids. Physical demands are attached plus push, pull, and grip up to 50 lbs., pinch up to 15 lbs., kneel, crouch, and crawl minimally. Equipment Used: Unit Specific
