Nurse Practitioner

Working under the supervision of the Carmel Richmond physician, the NP will provide primary and acute care services to Residents as a member of a health care team in primary care/care management role. This position will be part of the Delivery System Reform Initiative Program (DSRIP) responsible to provide clinical oversight on the 3pm-11pm shift of Residents who exhibit a change in medical condition. In addition, the nurse practitioner will provide onsite education to licensed nursing stalff of care pathways, assessment/evaluation training of the two projects, implementation of the Interact program and Palliative Care Program. The goal of these projects is to reduce hospitalization of residents.

* Identifies clinical care issues, providing direct primary care as an extension of the PCP for ill Residents. * Interacts with Residents/ families to keep them abreast of the Residents' condition . * Educates the residents/families on the Palliative Care Program. * Educates staff on Care Pathways and assessment/evaluation of clinical conditions and Palliative Care Program. * Performs health assessments, orders and performs diagnostic tests, interventions and skilled services within the scope of practice.

* Masters education * 3 years experience as an Nurse Practitioner (NP) * Licensed in New York State (NYS)
