Summary

Is responsible for quality nursing care of the residents within an assigned unit; contributes to the management of the unit under the direction of a Registered Nurse and/or Nursing Supervisor in support of medical care as directed by medical staff and pursuant to objectives and policies of the Home.

Responsibilities
  • Under the direction of a registered Nurse identifies residents needs and conditions and assist in developing a nursing plan of care, including rehabilitative and restorative activities and instructions in self help.
  • Sets long and short term goal.
  • Focuses plans on maintaining independence, socialization and self esteem.
  • Participates in resident plan of care committee meetings.
  • Admits and orients new residents and relatives.
  • Checks physicians orders, transfer and pre-admission information.
  • Reviews resident's medical history.
  • Verifies completion of requisition's and arrangement for diagnostic and therapeutic services, as ordered by the physician, from departments within the institution.
  • Orders prescribed medications, supplies and equipment.
  • Transcribes physicians orders to the medication sheets and treatment sheet.
  • Schedules appointments and arranges for transportation to other departments and clinics.
  • Has knowledge and awareness of resident's needs and does utmost to full fill these needs.
  • Attempts to fulfill spiritual and psychological needs of residents.
  • Continually re-evaluates and revises residents care plans to meet resident needs.
  • Infroms supervirsoe of all pertient changes in residents and special problems alloted to nursing care of the resdietns on a daily basis.
  • Makes rounds, takes and records vital sings as required.
  • Checks and administers prescribed medications, treatments and perform other professional services ordered or required. (these may include , but are not limited to , enemas, catheterizations, suction, inhalation therapy, administration of I.V. fluids, applying and changing dressing, bandages, packs, colostomy and drainage bags, etc. exercise, isolation set-up and care of deceased residents.)
  • Performs and/or supervises and assists with care of the residents according to outlined policy and procedures.
  • Gives emergency treatment when required and notifies the physician of the emergency.
  • Prepares equipment, accompanies and assists physician during treatment and examination of residents.
  • Notifies physician of changes in residents condition or any unusual or abnormal observations.
  • Assist with orientation, instruction and supervision of other nursing personnel.
  • Demonstrates nursing techniques and procedures and assist non professional nursing personnel in rendering care of the unit.
  • Continues professional growth through current literature, meetings, workshops and inservice programs in the home.
  • Gives and receives reports.

  • 1 year

License, Registration, and / or Certification Requirement:

  • Yes

Education Requirements

  • Graduate of an accredited School of Licensed Practical Nursing

Experience Requirements

  • 1-3 years of progressive nursing experience
