Summary
Nurse Coordinator (RN) describes a variety of specialized nursing positions that provide assistance in planning, coordinating, and delivering patient care for selected patient populations across the continuum of care using the nursing process, ensuring compliance with physician orders and established policies and procedures.
Responsibilities
Nurse Coordinators (RN) provide coordination and care of patients requiring assistance and support in accessing and managing their health care needs (e.g., cardiac and cancer patients). They collaborate with other health care providers to ensure continuity of care and coordination of a wide variety of patient services. They will use performance improvement methodology to design data collection systems / processes and identify opportunities to improve care / service for patients. Responsibilities include coordinating patient contact, patient education, and preparing patients/families for procedures/tests in collaboration with the health care team; collaborating with others to ensure continuity of care and coordination of a wide variety of patient services; identifying opportunities to improve patient care services; assisting clinic management to identify and develop operational needs for the clinic and to improve patient care services; and they may provide direct patient care.
Ideal Candidate
- 2 years of progressively responsible and directly related registered nurse work experience - 1 or more years' experience in clinical or ambulatory care nursing setting - Recent experience as Oncology Registered Nurse (RN) - Bachelor Degree of Science in Nursing (BSN) from an accredited college/university - Basic Life Support (BLS) for Health Care Providers from the American Heart Association - Valid California Registered Nurse (RN) license - Oncology Nursing Society certification highly preferred or committed to obtain one within 6 months of hire - Demonstrated skills in teaching, communication, coordination, and continuity of patient care
