* Responsible for rendering professional nursing care to patients in ther home by assessng, developing, implementing and evauating on assigned patients in keeping with agency policies and procedures, external regulations, and Advanced Practice Act. * Evaluating patients for recertification to the Hospice Program by performing face to face evaluations on patients in the homes, skilled nursing facilities and hospitals * Acts as liason among MD and other members of the heatlh care team to ensure effective coordination of receertification process. * Documentation of assessment, interventions and planning meet Hospice COP recertification guidelies * Maintains awareness of new/updated standards and incorporates them into his/her nursing practice throughout the continum of care.

* NJ license as a Advance Practice Nursed * NJ drivers license * Hospice experience preferred * Registered Nurse
