Summary
MEDIX is looking for a REGISTERED NURSE interested in doing CASE MANAGEMENT.
The registered nurse will be doing case management in the North DFW area.
This opportunity is a non-clinical role, but Registered Nurse skills are needed for the assessment, observation, evaluation, and coordinating of care plans.
Responsibilities
* Case Manager will visit nursing homes
* Nurse Case Manager assesses the care plan of patients
* Patient load will range between 8-10 patients a week
* Traveling to nursing homes within the DFW area
* Documenting and charting will be done 2/5 days a week remotely
Ideal Candidate
* Bachelor’s degree in Nursing
* EMR experience
* Ability to work in a team
* Ability to work independently
* Current RN license
* Current driver's license
* Current auto insurance
Compensation and Working Conditions
Working Conditions
- Monday-Friday - 8a-5pm - traveling to nursing homes for assessments - all other duties are performed remotely from home
