Summary
The purpose of positions in this classification is to manage home health or hospice patient care at a branch or headquarter location. Ensures for the provision of appropriate home health or hospice care to meet state and federal regulations and reimbursement criteria. May direct subordinate supervisory staff.
Responsibilities

Other duties may be required and assigned. • Monitors activities to ensure that branch staff remains in compliance with laws, regulations, guidelines, standards, policies, and procedures. -Takes appropriate action to ensure that the branch complies with applicable laws and regulations. Provides training, reference materials and guidance on regulatory guidelines for staff as needed. -Ensures for auditing of clinical documentation for compliance and addresses deficiencies. -Assists in the development of new policies and procedures. Communicates, clarifies and interprets policies and procedures for staff. -Ensures staff is following safety procedures and taking safety precautions. Investigates, completes and forwards documentation of work-related accidents and injuries. • Responsible for quality of patient care at assigned location(s). -Ensures that the staffing, resources, and supplies are available and appropriate to facilitate quality patient care. -Monitors the practice of the patient care team. Reviews and evaluates aspects of care and services. Ensures that physician orders are carried out properly. Ensures that the patient record and care team documentation is complete and in compliance. -Reviews key metrics including quality measures, staff productivity, patient census, visits, episodes, case mix, new admissions, discharges, etc. Ensures that operational metrics are monitored and on target. Takes appropriate action for outliers. -Holds weekly case conferences to go over work assignments and specific patient cases. • Responsible for the management of resources at assigned location(s). -Assists in the development of a branch operational plan to meet patient, financial, quality, service, and growth targets. -Assists in reviewing the preliminary budget for the assigned area and provides input. -Monitors and manages expenses including staff, travel, overtime, supplies, etc. Ensures staffing levels remain within budget. -Reviews and monitors revenues and recommends appropriate adjustments in staffing. -Ensures account codes and allocations are accurate. -Notifies leadership of any financial concerns. • Collaborates with departments on growth strategies, marketing, and educational opportunities. Actively networks to promote the organization and enhance the organization’s reputation; to strengthen and build positive relationships; and to increase our visibility and value within the community. Calls on key referral sources to communicate HHU’s mission and services. • Supervises the patient care team (e.g. RN, LPN, CNA, Aides, COTA, OT, PT, OTA, Spiritual Counselor and Social Services staff) and office personnel. Reviews and evaluates requests for service and assigns client cases to appropriate staff members. Ensures for adequate staffing levels. Provides on-going direction and guidance to staff. Ensures staff complete required visits in a timely manner. Follows-up with staff to ensure that missed visits are handled in accordance with proper procedures in a timely manner. Verifies that the patient plan of care is followed. Audits charts to ensure for quality of care and appropriate documentation. • Communicates with and updates supervisor on activities and issues within branch on a regular basis including complaints and disciplinary actions.• Monitors the practice of patient care team on a regular basis either through observation of patient care in joint or individual visits or through the regular review of case documentation to ensure quality of care and maintenance of standards of performance. Consults with staff individually and collectively regarding patient care and service needs. • Performs personnel-related functions: ensures personnel policies and procedures are communicated and followed; tracks employee attendance and ensures for the accurate documentation of hours worked; reviews and approves requests for leave; assists in the interviewing and hiring of staff; monitors and documents employee performance on an on-going basis and provides regular performance feedback to staff; recommends and documents personnel actions; documents staff deficiencies and takes appropriate action including coaching, verbal counseling, and implementation of progressive disciplinary measures and; conducts performance reviews. • Responds to concerns or complaints from patients, family members, or health care institutions. Investigates and documents incidents. Recommends resolution. Takes disciplinary action as necessary. Conducts follow-up as necessary. • Ensures for staff safety. Provides safety instruction and ensures staff are following safety procedures and taking safety precautions. Identifies and notifies the Safety Committee of potential safety hazards at branch or headquarter locations. Investigates, completes and forwards documentation of work-related accidents and injuries. Tracks leave and light duty time. • Travels to patient homes and performs patient care and patient assessment duties. • Facilitates and attends staff, clinical management, hospice, IDG meetings and clinical development meetings, as appropriate. • Serves as a liaison to area hospital staff, area medical director, and area referral sources. • Responsible for the management of resources at area location. Prepares a preliminary budget for review for the assigned area. Monitors and tracks expenditures, overtime, and other costs. Ensures staffing levels and expenditures remain within budget. Notifies leadership of any budget concerns. • Ensures for staff development and training. Identifies deficiencies and provides one-on-one education and training to staff. Recommends staff for seminars and provides preliminary approval of training requests. Encourages professional development through case conferences, record evaluation, planning and leading group conferences and staff participation in educational seminars and training offerings. • Coordinates and/or participates in community and marketing functions to promote Home Health United.
Ideal Candidate
Minimum Education and Experience: Bachelor’s degree in Nursing, Occupational Therapy, or a related clinical field with three to five years progressively responsible clinical experience that includes home health and/or hospice. Medical Social Work requires a Master’s degree.Current State of Wisconsin licensure in field of study is required (e.g. RN, LCSW, APSW, or OT licensure). Requires a minimum of two to three years prior clinical supervisory experience where supervision was a primary essential job function. Requires prior experience assigning and evaluating work and assisting in the hiring, firing, and training of employees as the direct and immediate supervisor.Prior managerial experience strongly desired. Requirements: This position requires travel to patient homes and meeting locations.Must maintain a valid Wisconsin driver’s license.Must maintain an acceptable driver’s record per company policy.Must maintain proof of current auto insurance.Must have access to a vehicle in good working condition. Knowledge, Skills & Abilities: Knowledge and principles of home health care. Knowledge of symptom management, pain control, disease management, telehealth and self-management. Knowledge of available community resources.Knowledge of federal and state regulations as relate to the provision of home health services. Knowledge of Medicare and Medicaid regulations, requirements and reimbursement criteria. Knowledge of Home Health United Policies and Procedures. Handles sensitive and confidential material in a professional manner and in accordance with legal requirements relating to the safeguarding of confidential information/communications. Physical Demands: This position rates as MEDIUM WORK as defined by the Dictionary of Occupational Titles
