Summary
Under the direction of the supervisor, the Program manager will work with foster children, their family/guardian, and foster parents to ensure the provision of comprehensive case management services. Must be able to conduct intake placements, advocate for clients in court, conduct home visits, assess needs of children and families, transport clients as required, and complete required paperwork.
Responsibilities
* Respond to all calls requesting placements and screen for appropriateness. * Complete Intake information packet. * Confirm foster parent availability and arrange for pick-up. * Monitor transfer of child(ren) from natural parent/ guardian to Foster Parents (FP). * Maintain regular contact with the FP. * Maintain daily/monthly documentations. * Arrange and monitor discharge of placements. * When child is in care, make at least weekly visits to foster home; or, quarterly consultations with FP. * Attend monthly child and family team meetings. * Attend IEP meetings as applicable. * Assist with or conduct workshops; assist with child care during trainings. * Respond to crisis. * Be available during the work week for crisis calls with your consumers and families. * Be available to all FP while serving a rotating weekend on call shift. * Attend court, DSS, or School meetings as necessary. * Keep MIS system updated in a timely manner. * Complete critical incident reports. * Travel as necessary. * Monitor supplies with all FP (i.e., car seats). * Make requests for additional supplies, as needed, to Program Coordinator. * Maintain neat storage room for supplies. * Staff Responsibilities include; Attend weekly supervision with Program Coordinator, Attend monthly individual supervision, Attend two times a month case supervision, Attend Program Team meetings, Attend community meetings when needed, Successfully complete all required agency trainings/ in-services, Complete all required paperwork in an accurate and timely manner, Keep work area clean, Conduct monthly reviews of files to ensure completeness, Other duties as assigned.
Ideal Candidate
* Education – BA/BS degree, from an accredited College or University, and one year experience in a Human Services or related field of study. * Experience - Detail oriented and able to manage multiple assignments independently. Knowledge of state and federal laws, regulations, and practices related to Family Foster Care Program. * Skills – Excellent interpersonal, written, and verbal communication skills. Skilled ability in computer systems including PC software, spreadsheets, Word, Access, PowerPoint and other applications. Ability to maintain good rapport with community partners and supports, other employees, and demonstrate constructive problem-solving skills. * Physical Demands – Must be capable of lifting or moving office supplies and materials up to 20 pounds. The office may be a busy and noisy facility from time to time. * Other – Must meet state criminal background check requirements. * Travel - Must be able to meet requirements for Eckerd’s Auto Insurance and be able to drive for business purposes. Incumbent must possess and maintain a valid NC driver’s license and have reliable transportation. Required to travel frequently between office and locations within the service area, attend meetings and conferences required to fulfill the obligations and responsibilities of the position.
