Summary
The clinical pharmacist performs a variety of job functions relating to the direct care of patients, education of physicians and staff regarding clinical information and implementation of clinical programs that involve patients and drug therapy.
Responsibilities
* See patients in clinic to be able to change, titrate and monitor therapy and conduct CMRs. * Review patient drugs in person or telephonically through the MTM program. * Develop/ maintain pharmacy programs i.e., Medication Therapy Management, Adherence initiatives, generic conversion initiatives, polypharmacy etc. * Participate in development of educational lectures and materials for CareMore clinical staff and patients. * Answer inquiries from physicians and nurse practitioners regarding clinical appropriateness, current guidelines and new drugs. * Answer inquiries from sales team, and members related to formulary and prescription benefit. * Work with local leadership regarding pharmacy metrics and design/implement programs as needed and to reduce pharmaceutical costs to members and the organization. * Travel to providers to educate drugs related to cost savings and generics. * Understand and present reports on drug utilization and cost to providers and regional management. * Adhere to all Medicare part D policy and procedures. * Some local travel required. * Minimal supervisory duties; possible preceptor of pharmacy students.
Ideal Candidate
* 2+ years of ambulatory care pharmacy experience or residency in lieu of work experience; or any combination of education and experience, which would provide an equivalent background. * Degree in Doctor of Pharmacy (PharmD.) * CA Board of Pharmacy Pharmacist License required. * Managed Care experience preferred. * Spanish speaking a plus
