Physical Therapist Case Manager

* Assessment skills: initiates appropriate assessment for diagnosis * Direct client care: continuous assessment of patient needs and functional deficits, progresses plan of care, initiates appropriate referrals to other disciplines and provides skilled care * Operates under appropriate authorizations from MD and payer - any changes in interventions are initiated after appropriate MD order * Clinicians demonstrate required patient educator competencies consistent with their scope of practice to ensure that patient education requirements are met * Case management as necessary * Documentation, follows regulatory guidelines (state, federal) * Teamwork and organization

**Education** * BS or MS in PT from an APTA Accredited Program **Qualification/Skills** **Experience** * 2-3 years experience in SNF, acute care setting * Prefer some hospice experience **Skills** * Ability to assess age related general medical and environmental conditions and needs of clients * Good verbal and written communication skills * Ability to function independently as an interdisciplinary team member * Must be adaptable and flexible, willingness to cross over geographic areas in time of need * Excellent time management and organizational skills * Ability to articulate the value of the PT discipline * Basic computer skills * Understanding of and sensitivity to end of life issues and palliative care as it applies to physical therapy goals * Preferred affiliation with professional organization MNAPTA/APTA **Licensure/Certifications** * Licensed to practice Physical Therapy in the State of MN * CPR every two years **Work Schedule – Additional Position Details** * $10,000 sign on bonus. * Full Time or Part Time - Can be flexible on the FTE based on preference * 40 hours/week * Day Shift * Monday-Friday * Non-Contract
